ResolutionCare seeks an inspired, experienced, full-time Registered Nurse / Associate Clinical Director to join our community-based palliative care organization, supporting Mendocino Care Network in Ukiah/Willits and interdisciplinary teams in Eureka and other geographic regions along with and as directed by our Clinical Director. Applicants have the ability to choose to live in or near either Ukiah/Willits or Eureka, with the flexibility for frequent remote work. Joining our team provides an opportunity to redefine medicine as we restore the tradition of person-centered home based care through the soulful application of technology. Our incredible transdisciplinary team supports people and families to chart their own path on their own terms in the face of serious illness.

By using a combination of on-the-ground home care with telemedicine, mobile health applications and an integrated technology platform, we are able to bring the right care, at the right time, in the right place for those we serve. 

The RN Associate Clinical Director will contribute to the development of our growing clinical program while providing training and mentorship to our staff. The Associate Clinical Director will provide direct patient care as well as program leadership for our teams in Mendocino County and beyond. Our ideal candidate is an experienced registered nurse with expertise in leadership, program innovation and clinical training and education.

A CHW provides support to seriously ill individuals and their families in collaboration with an interdisciplinary care team. Our ideal candidate is someone with two years caregiving, home health or skilled nursing facility experience. End of life experience is preferred.
