Mendocino Care Network

The Mendocino Care Network is a new community-based Palliative Care Program for all of Mendocino County. This program began service on August 1, 2017. The Mendocino Care Network has joined ResolutionCare to expand Palliative Care Services in Mendocino County.

Our goal is to provide support to all members of our community who need help dealing with the challenges they face with serious illness. Our care team will meet with you and your family to hear what you need, then work to help improve the quality of your life. We will communicate closely with your medical providers to coordinate your care and relieve your symptoms. We will be on call 24 hours a day to back you up during difficult times.

Associate Clinical Director Position

ResolutionCare seeks an inspired, experienced, full-time Registered Nurse / Associate Clinical Director to join our community-based palliative care organization, supporting Mendocino Care Network in Ukiah/Willits and interdisciplinary teams in Eureka and other geographic regions along with and as directed by our Clinical Director. Applicants have the ability to choose to live in or near either Ukiah/Willits or Eureka, with the flexibility for frequent remote work. Joining our team provides an opportunity to redefine medicine as we restore the tradition of person-centered home based care through the soulful application of technology. Our incredible transdisciplinary team supports people and families to chart their own path on their own terms in the face of serious illness. 

ResolutionCare isn’t centered around the technology. We simply employ 21st century tools to provide true person-centered care. By using a combination of on-the-ground home care with telemedicine, mobile health applications and an integrated technology platform, we are able to bring the right care, at the right time, in the right place for those we serve. 

The RN Associate Clinical Director will contribute to the development of our growing clinical program while providing training and mentorship to our staff. The Associate Clinical Director will provide direct patient care as well as program leadership for our teams in Mendocino County and beyond. Our ideal candidate is an experienced registered nurse with expertise in leadership, program innovation and clinical training and education.

Mendocino Care Network/ResolutionCare seeks a part-time Community Health Worker to join our community-based palliative care program. This is an opportunity to work with a growing organization that is pioneering the use of telemedicine to support seriously ill people in their homes. 

A CHW provides support to seriously ill individuals and their families in collaboration with an interdisciplinary care team. Our ideal candidate is someone with two years caregiving, home health or skilled nursing facility experience. End of life experience is preferred. Lynn Dress-Meadows, PA

Lynn Dress-Meadows,

Physician Assistant

 Program Director

Lynn and her husband Bob Dress have been residents of Greenfield Ranch in
Mendocino County since 1972. She is excited to be bringing Palliative Care to
the entire community. She loves having conversations with people and
families about their wishes and goals, then working with the care team to
improve life and reach those goals.

Lynn is a gardener, grandmother, watercolor painter and volunteer community
organizer.  She is on the board of Hearthstone Village, a non profit
supporting her 26 daughters at the Reveil Matinal Orphanage in Haiti. Her
battery gets charged by spending time on the Ranch and at her home on the
Sea of Cortez in Mexico.

Lynn graduated from the Stanford University/ Foothill College Physician
Assistant Program and the Stanford University /San Jose State University
Midwifery Education Program.

Faith Dayton

Faith Dayton, LCSW

 Social Worker

Faith Dayton has been working as a medical social worker since 2013. She completed the UCSF Interprofessional Palliative Care Education program in 2018, and has worked in palliative care in both the in-patient and out-patient settings since 2013. She has completed training in POLST education and advanced care planning, and is literate in medical case management.

She has a passion for assisting patients in navigating the difficult health care and social service systems, and empowering them to reach their personal goals.

Faith values compassion and social justice. She enjoys helping people and their families in having better quality of life while they confront difficult medical conditions and disease.

Jenny Grissom

Community Health Worker

Jenny has worked in medical case management since 2014. She met Lynn Meadows, PA while working in an inpatient facility, and made the move to coordinating outpatient care because she loved the mission of helping provide palliative care services to her community. Jenny has a BA in Psychology and is currently in a Masters of Social Work program through Humboldt State University. She loves travel, spending time outdoors with her children, and a good movie.

Dana Landes

Dana Landes

Care Coordinator

A transplant from the Bay Area, Dana moved to Humboldt County to be close to her aging parents.  With a degree in psychology and wide range of professional and personal experience, she is excited to bring her unique gifts and talents to her position of Patient Care Coordinator.

Dana has a compassionate, tender heart and is hopeful to cultivate and strengthen these aspects of herself.  She couldn’t be happier to spend the best hours of the day and the best years of her life serving this team.  She whole-heartedly believes in the goals and values that this special group of people is fostering and creating.  She stands with ResolutionCare as it brings amazing resources to our community while educating and being an example to others.

Outside of work, Dana enjoys music and dance foremost! She continues to learn and practice drums while enjoying the transformative effects of listening to great music.  She is fascinated with the topics of metaphysics, science and philosophy.  She also enjoys the outdoors, hiking and yoga, spending time with friends and family. Her cats, Zildjian and Maybe are her sweet four legged companions.
